How to set up health insurance for a startup

To set up health insurance for a startup, count employees, set a contribution budget, compare group coverage with any reimbursement arrangement you qualify for, and confirm enrollment rules before announcing a start date.
Work through the decisions
- List employees, work states, dependents you may cover, and your target start date.
- Decide what the company can contribute every month and how costs change as you hire.
- Get local group quotes and review SHOP eligibility. HealthCare.gov lists SHOP requirements.
- Ask whether a QSEHRA or ICHRA is suitable. HealthCare.gov compares small-employer reimbursement options.
- Give employees the written plan information, deadlines, and a contact for enrollment questions.
Check the employee side
A low employer budget can still leave employees with high premiums or out-of-pocket costs. Ask for examples covering an employee alone and an employee with dependents. If using an HRA, check individual-market availability where employees live and how the arrangement interacts with premium tax credits. HealthCare.gov explains ICHRA coverage requirements.
