Electronic Signature Consent

Denta, Inc. ("Denta") provides its agreements, notices, and records electronically. This consent explains what that means and what your rights are. Please read it before you sign anything else, because every signature you give in onboarding relies on it.
1. What this consent covers
By accepting this consent, you agree that Denta may provide the following to you electronically rather than on paper:
- The Denta Terms & Conditions and Privacy Policy
- The Administrative Services Agreement and Business Associate Agreement
- The Plan Document, Adoption Agreement, and HIPAA Plan Sponsor Certification
- The Claims Funding Authorization
- The Summary Plan Description and Notice of Privacy Practices
- Statements, invoices, claim notices, and any other record or notice relating to your account or your plan
This consent covers all of these and any future records of the same kinds, until you withdraw it.
2. Consent to electronic records and signatures
You agree that:
2.1 Electronic records satisfy any legal requirement that a record be delivered to you in writing.
2.2 Your electronic signature has the same legal effect as a handwritten signature, and an electronic record has the same legal effect as a paper original, under the federal ESIGN Act and applicable state law.
2.3 You will not contest the validity or enforceability of an agreement solely because it was signed electronically.
3. What counts as your signature
When you sign in the Denta onboarding flow, your signature is the record Denta creates at that moment: your typed full name, your title, your affirmative acceptance of each document, the date and time, and a cryptographic fingerprint of the exact text you were shown.
Denta keeps that record and will provide you a copy on request.
4. Your right to paper copies
You may request a paper copy of any record covered by this consent, free of charge, by emailing admin@trydenta.com or writing to the address at the end of this consent. Requesting a paper copy does not withdraw your consent.
5. Your right to withdraw consent
You may withdraw this consent at any time by emailing admin@trydenta.com. Withdrawal takes effect once Denta has had a reasonable time to act on it.
Withdrawing consent does not affect the validity of anything you signed or received before the withdrawal took effect. Because Denta's services are delivered electronically, withdrawing consent may mean Denta can no longer provide them, and may end your use of the Services.
6. Keeping your contact details current
Denta sends electronic records to the email address on your account. You are responsible for keeping that address current and able to receive our messages. Update it in your account settings or email admin@trydenta.com.
7. What you need to access these records
To receive and keep electronic records you need:
- A current version of a common web browser
- An active email account, and software able to read email
- A device with internet access
- The ability to view and save PDF files
- Enough storage, or a printer, to keep a copy for your records
If Denta changes these requirements in a way that materially affects your ability to access records, we will notify you and give you the chance to withdraw this consent without charge.
8. Confirming you can receive them
By accepting this consent electronically, you confirm that you can access this document in the form in which it was presented, and that you can receive and keep records at the email address you gave us.
9. Authority
If you are accepting on behalf of a company, you confirm that you are authorized to consent on its behalf and to bind it to agreements signed electronically.
